You have been provided with these Terms of Business prior to booking an appointment with Carine Henry RD, Paediatric Dietitian.  All appointments times and dates are negotiated and agreed at the time of booking.  You will receive information about the appointment via email, which you are required to confirm together with acceptance of these terms and conditions. Confirming a booked appointment assumes acceptance of these terms and conditions.  Do please read them carefully and discuss any questions you have before the appointment.

What is a Registered Dietitian?
Registered Dietitians are health professionals that are qualified to assess, diagnose and treat diet and nutrition-related problems, relying on integrating nutritional knowledge, public health guidance and evidence-based decision-making. Paediatric Dietitians have received specific training on the needs of infants and children. Only those professionals registered with the Health Care and Professions Council can use the title Dietitian. Registration details can be found here.

Health Insurance
Most health insurance providers reimburse dietetic fees for those referred by their Paediatrician or Specialist. Some insurers will reimburse fees for GP referrals, but most will not. Carine Henry is registered with most health insurance providers with the HCPC Registration Code DT5478. Do seek an authorisation code from your insurer prior to the appointment if you intend seeking reimbursement.

Payment of Fees
This is your responsibility, not the insurer.  Payment terms are 14 days net from the date of the appointment.  Late payment may attract interest at the statutory rate of 8% above the Bank of England base rate.

Cancellation
Failure to attend a booked appointment or a cancellation less than 24 hours from the time of the appointment will incur a charge of 50% of the agreed fee. Health Insurance providers will not pay fees incurred for late cancellation and you will be liable.   Published fees can be viewed at www.paediatricnutrition.com/fees

Confidentiality, Privacy and Data Protection
Confidentiality refers to the respect and privacy a health professional is obligated to have towards your information. Following appointments, your information is kept in accordance with data protection legislation and is not used or shared, except with your GP, without your express permission. This includes medical records, letters, results or written reports containing dietetic, medical and other personal or contact information. Carine Henry is compliant with the ICO Data Protection, Registration No. ZA133818

Child Safeguarding
If a child is thought to be at serious risk of harm, and sharing confidential information believed to be necessary to protect the child from harm; consent to share information is not required.
